Key Responsibilities

  • •Oversee day-to-day operations of the CDS Program across the Sydney and Southwest Sydney region
  • •Provide operational leadership to the team
  • •Act as a positive role model to ensure participants are supported to live a fulfilling and meaningful life in the community
  • •Work with various external stakeholders including the NDIA, other disability service providers and funders, families, and health professionals within the circle of participants
  • •Ensure high quality outcomes are achieved for the service in line with client care plans and organisational requirements
  • •Plan and complete service agreements and schedules of supports with participants
  • •Support NDIS participant plan reviews
  • •Roster CDS shifts
  • •Recruit, onboard, support, train and supervise CDS staff
  • •Lead practice support relating to complex work
  • •Monitor intake and capacity
  • •Facilitate team meetings and practice discussions
  • •Motivate, support, and mentor team members
  • •Monitor resources and make sure they are always sufficient
  • •Ensure all client paperwork is completed to a high standard and signed off
  • •Make client appointments and communicate appropriately
  • •Work collaboratively with the Clinical Care Consultant, APO representative and wider leadership and CDS teams
  • •Maintain a high level of program quality and driver continuous improvement
