Key Responsibilities

•Provide intensive case management, assessment, planning, monitoring and crisis intervention
•Work one-on-one with clients in outreach settings
•Assist clients to build independence and achieve personal goals
•Maintain accurate client records and ensure compliance with privacy and legislative requirements
•Establish and maintain service statistics and data to support program development
•Work closely with prison staff, corrective services, government agencies and community organisations
•Conduct case management in consultation with the Prisoners Support Unit, prison management and other relevant partners
•Promote the program within the community and contribute to ongoing service development
•Participate in regular planning and review sessions with the program team
•Ensure day-to-day service delivery aligns with The Salvation Army’s mission and policies
•Complete administrative tasks, including forwarding accounts for processing
