Key Responsibilities
- Provide intensive case management, assessment, planning, monitoring and crisis intervention
- Work one-on-one with clients in outreach settings
- Assist clients to build independence and achieve personal goals
- Maintain accurate client records and ensure compliance with privacy and legislative requirements
- Establish and maintain service statistics and data to support program development
- Work closely with prison staff, corrective services, government agencies and community organisations
- Conduct case management in consultation with the Prisoners Support Unit, prison management and other relevant partners
- Promote the program within the community and contribute to ongoing service development
- Participate in regular planning and review sessions with the program team
- Ensure day-to-day service delivery aligns with The Salvation Army's mission and policies
- Complete administrative tasks, including forwarding accounts for processing
