Key Responsibilities
- Build trust-based relationships with clients
- Provide case management according to TSA models
- Complete assessments of needs, risks, and vulnerabilities
- Conduct ongoing reviews from intake to exit
- Develop collaborative case plans with clear goals
- Support clients to access services and community supports
- Monitor progress and adjust plans as needed
- Maintain accurate client records using TSA systems
- Identify risks and escalate to supervisors
- Recognize service gaps and contribute to improvement
- Participate in network meetings and professional development
- Engage in regular supervision and case reviews
- Participate in on-call roster and night work
