Key Responsibilities
- Conduct assertive outreach to engage potential clients and local intermediaries
- Build rapport and trust with older people from diverse backgrounds
- Respond to enquiries and intake applications promptly, providing warm handovers where required
- Assist clients to reconnect with My Aged Care or alternate providers when circumstances change
- Ensure clients receive the right supports to achieve their goals and improve wellbeing
- Maintain regular check-ins once services commence
- Manage a full caseload within organizational guidelines
- Contribute to continuous improvement across program policies and practices
- Participate in WHS activities and promote a safe work environment
